This course relies on very outdated information. The more room you add to the shield, the more areola will enter the tunnel. When I size NICU mothers I do not automatically add 4mm to their measurement. That will cause swelling and sometimes pain. Medela mentions Lactahub as a resource and there is a video explaining how to tell if the flange is too large. The IBCLC mentions swelling is a sign the flange is too large and she goes down in size and tells the mother as swelling decreases with the better fitting smaller flanges she will have less pain.
